If you would like to speak directly to Dr. Della Valle prior to your first office visit, please call to let us know you are sending a copy of your recent X-Rays (on a CD) and your completed new patient intake form (see below). The doctor will then review the information and call you to go over your case prior to your office visit. We can arrange a simple phone call or a secure video conference via the Internet.
